Become a Member

Joining the NDA is one of the best investments you can make! 

If you have been a member in the past several years but let your membership lapse, you do NOT need to fill out a new membership application. Simply contact the NDA to get the amount of your dues for the current year. Once the dues are paid, your membership will be renewed.



There are two ways to join:

1. Complete the online ADA membership application; 
2. Download and complete the ADA membership application PDF form , then save the form to your computer and upload it.

Once submitted, the NDA will process your application and contact you about the amount of your dues.  New applicants who prefer to mail in a membership application can download the file and send to the following address:

Once the application has been processed, an email will be sent with the amount of your dues.
